PAINFUL OPHTHALMOPLEGIA IN CAROTID-CAVERNOUS FISTULA

نویسندگان

چکیده

Ophthalmoplegia is defined as a consequence of multiple cranial nerve paralysis innervated the extraocular muscles. The impairment ophthalmic branch trigeminal will create painful condition. Numerous underlying mechanism have been proposed related to ophthalmoplegia and carotid-cavernous fistula one main causes with sight life threatening outcome if left untreated. Carotid-cavernous arises an abnormal communication between carotid artery cavernous sinus. Important signs symptoms may include red eye, proptosis, ophthalmoplegia, headache vision loss. Neuroimaging angiography are mandatory in diagnosing disease. Systematized approach early recognition evaluation lead prompt management avoiding morbidity mortality severe cases.

Ophthalmoplegia in carotid cavernous sinus fistula.

The aetiology of ophthalmoplegia in 15 patients with carotid-cavernous sinus fistula is discussed, and the clinical findings are correlated with angiographic and orbital CT appearances. After closure of the fistula the majority of patients with generalised ophthalmoplegia recovered full ocular movements rapidly, while patients with an isolated abduction weakness required much longer to return t...

Spontaneous carotid-cavernous fistula with ophthalmoplegia and facial palsy.

Spontaneous carotid-cavernous fistulae are uncommon. They are seen mostly in association with hypertension, atherosclerosis or intracranial aneurysm (Bickerstaff, 1970). Facial palsy was noted by Walsh (1957) in a case of traumatic carotid-cavernous fistula which recovered in one year. Involvement of the third, fourth, fifth and sixth cranial nerves has been frequently noted (Walsh, 1957; Sattl...

Painful and painless ophthalmoplegia with cavernous sinus pseudotumour.

Acquired isolated ophthalmoplegia in childhood has many potential causes. Although other ophthalmological or clinical features may aid lesion localisation, the absence of these does not preclude structural pathology. Two cases of cavernous sinus pseudotumour presented as ophthalmoplegia with and without pain. Magnetic resonance imaging of the cavernous sinus revealed the presence of enhancing t...

Glaucoma Management in Carotid Cavernous Fistula

Carotid cavernous fistulas (CCF) are vascular communications between the carotid artery and the cavernous sinus. Ophthalmologists are called to diagnose and manage the condition in cases that present with ocular features. A 73-year-old female was referred to our glaucoma center clinic. Eight years before, she had started receiving medication for glaucoma and had undergone laser iridotomy, but a...

Painful fourth cranial nerve palsy caused by posteriorly-draining dural carotid-cavernous sinus fistula.

A 65-year-old man with a dural carotid-cavernous fistula (DCCF) presented with sudden onset of painful trochlear nerve paresis. Typical signs of DCCF including conjunctival arterialization, chemosis, and proptosis did not become manifest until 4 months later. This unusual presentation of DCCF was caused by drainage of the fistula posteriorly into the inferior petrosal sinus with low flow. With ...
